To evaluate the effect of a pharmacist-led intervention on self-management practices among hypertensive-diabetic patients receiving care in a Nigerian tertiary hospital.

A pharmacist-led intervention among hypertensive-diabetic patients receiving care in a Nigerian tertiary hospital improved adherence to hypertension self-management practices including low salt diets, weight management practices, more involved in physical activity and adherence to medications.

3) Which were the more important methods you used in this work? If it is not a traditional method, you can briefly explain the concept of that methodology.

a) Identification of self-management gaps
b) Incorporating these gaps into an educational intervention program
c) Randomised controlled trial of 231 patients who had hypertension and other comorbidities. The control group received usual care whilst the patients in the intervention group received the educational intervention program

4) What did you learn from this paper, what was your take-home message?

It is important that educational programs be organised for patients with chronic diseases, as this helps them actively take part in self-care.
